IRMAA Lookback

Will your income raise your Medicare premiums?

Medicare looks back two years at your income. A Roth conversion, a home sale or a large RMD this year can raise your premiums in two years’ time. Check your tier, and how close you are to the next one.

How you filed your 2024 taxes

That’s your adjusted gross income (Form 1040, line 11) plus any tax-exempt interest (line 2a). Married filing jointly? Use the total on your joint return. 2026 premiums are based on 2024 returns.

How this is calculated

IRMAA (income-related monthly adjustment amount) is an extra charge on Part B and Part D premiums for people with higher incomes. We use the 2026 brackets and amounts published by Medicare.

Social Security sets your 2026 premium from your 2024 tax return, two years earlier. If it doesn’t have that return yet, it uses the one before and corrects the amount later.

The Part D amount is added on top of your drug plan’s premium, and only if you have Part D or a Medicare Advantage plan with drug coverage. For a married couple filing jointly, each spouse on Medicare pays it, based on your combined income.

The planning room figure assumes whole-dollar incomes. Brackets move with inflation each year, so treat it as a guide for future years.
